The call is coming from inside your underwear

Johnny Mercy receives a threatening call from a blackmailer who demands he perform humiliating acts, including giving himself wedgies and rolling in the mud. Despite his embarrassment and tears, Johnny complies out of fear of exposure. The blackmailer orders him to strip and intensify the wedgies until his underwear nearly tears. Johnny obeys, driven by terror of the consequences.
