Penis enlargement: what works

Pills, pumps, extenders, jelqing, fillers and surgery, checked one by one against the studies and the urology guidelines. Spoiler: not much, and not by much

Every inbox has seen the spam: three inches in thirty days, guaranteed. The industry behind it is huge, and the science behind it is tiny. A 2020 systematic review that went looking for every decent study on enlargement in men with a normal penis found 17, all of poor quality. Here is what those studies, and the guidelines urologists actually follow, say about each method.

Pills, creams and supplements

Nothing taken by mouth or rubbed on the skin has ever been shown to make an adult cock bigger. That part is easy. The worrying part is what the pills contain. When researchers bought 91 samples of "herbal" or "all natural" sexual enhancement supplements in US convenience stores and petrol stations and analysed them, 74 of them, 81%, contained prescription erection drugs or close chemical copies, none of it on the label. Some held more sildenafil or tadalafil than the strongest approved dose, and only a handful warned against mixing them with nitrates.

The FDA keeps a long, constantly updated list of these products and warns that they are likely to be contaminated with hidden ingredients and can lead to hospitalisation. That matters for anyone on heart medication containing nitrates, and for anyone who likes poppers, which belong to the same family: together with erection drugs they can make blood pressure fall sharply. The pill may give you a firmer erection, which is the hidden drug working. It will not give you a single extra millimetre.

Pumps and jelqing

A vacuum pump pulls blood into the shaft and makes it look fuller for a while, which is why some men love them in bed. As a way to grow, it fails. In 37 men who pumped three times a week for six months, stretched length went from 7.6 cm3 in to 7.9 cm3.1 in, a change that was not statistically significant. The systematic review and the European guidelines reach the same conclusion: no meaningful increase in size. The fun side of pumps is in Cock rings and pumps.

Jelqing, the milking stroke repeated hundreds of times a day, has a huge online following and no trial at all. Not a bad trial: none. The closest thing is a 2015 study comparing men who were very anxious about their size with men who were not: the anxious men were far more likely to have tried jelqing, pumps or stretching, and reported poor success. No study has ever weighed its risks against a benefit, because no benefit has been shown. If it hurts, bruises or leaves you numb, that is your cock telling you something.

Extenders: the only thing with numbers

A traction extender is a frame that holds the soft penis under gentle, constant pull for hours a day; the idea is that tissue kept under tension slowly lengthens. It is the one non-surgical method with real data, and the data come with a catch: the hours.

So: somewhere between one and two centimetres, mostly measured soft or stretched, after months of wearing a contraption under your clothes for a good part of every day. Shaft girth did not change in any of the three. Gains held after the men stopped, at least for the few months they were followed. None of these trials had a control group, all were small, and in the 2020 review the soft gains stayed under 2 cm0.8 in. The European guidelines call extenders effective by roughly 1.3 cm0.5 in to 2.3 cm0.9 in, on low-quality evidence, with a weak recommendation.

The small print

Most of the men in these trials were not small to begin with. In the largest one, 163 men came in complaining about their size, all of them got counselling first, and only 54 still wanted the device afterwards. Satisfaction at the end was described as "modest".

Fillers and injections

Injections add girth, not length, and here there is a sharp line between medical fillers and DIY oils.

Hyaluronic acid, the same gel used in lips, has been tested properly. In a randomised trial in Korea, 64 men gained an average of about 2.3 cm0.9 in in circumference after 24 weeks, with no serious side effects. The European guidelines report girth gains of 1.4 cm0.6 in to 3.8 cm1.5 in and give it a weak recommendation. The gel is not permanent: keeping the result needs retouches and new injections. And the 2020 review still flagged injectables as a whole for a high complication rate.

Paraffin, silicone, vaseline and other oils are a different story. The body walls them off as foreign material and forms hard, lumpy masses called paraffinomas. A review of 124 cases found pain, swelling, ulcers and deformity, with most men needing the material cut out and the skin rebuilt. Injected silicone can also travel in the blood and reach the lungs. The European guidelines recommend strongly against these injections, listing complications up to Fournier's gangrene, a flesh-destroying infection. If someone offers to make your cock thicker with a syringe at a party or in a back room, the answer is no.

Surgery

The classic lengthening operation cuts the suspensory ligament, the band that ties the base of the penis to the pubic bone, so more of the shaft hangs forward. It does not grow anything; it moves out part of what was already there. In a British series of 42 men, stretched length increased by 1.3 cm0.5 in on average, with results ranging from a loss of 1 cm0.4 in to a gain of 3 cm1.2 in, and only 35% of the men were satisfied. A review of 34 surgical papers found typical gains of 1 cm0.4 in to 2 cm0.8 in in length and frequent trouble: deformity, paradoxical shortening, ugly scars, lumps, migrating filler and sexual dysfunction.

The American Urological Association has said since 1994, most recently reaffirmed in 2018, that cutting the suspensory ligament to lengthen the penis has not been shown to be safe or effective, and the same goes for fat injections to add girth. The European guidelines link lengthening surgery to a significant rate of complications, and the 2020 review called surgery and injectables a last option, unethical outside clinical trials.

Most men asking are already normal

This is the finding that runs through every paper. In the review, the vast majority of men seeking enlargement had a normal-sized penis, and when counselling was offered, most came to see that and lost interest in treatment. Clinicians call the persistent version of the worry "small penis syndrome", and it can be part of anxiety or body dysmorphic disorder. A 2021 review concluded that most men seeking surgery have normal cocks and would be better served by accurate numbers.
